THIS NOTICE DESCRIBES HOW HEALTH INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION.

Honolulu DNA Paternity Testing understands the importance of privacy and is committed to maintaining the confidentiality of your health and identifying information. We create and maintain records related to DNA collection, identity verification, testing services, and results, and we may receive such information from other authorized sources.

We use this information to provide DNA testing services, comply with legal and accreditation requirements, obtain payment for services when applicable, and operate our business in compliance with federal and state law.

We are required by law to:

  • Maintain the privacy of your Protected Health      Information (PHI)

  • Notify affected individuals following a breach of      unsecured PHI

We collect health and identifying information about you and store it in secure electronic and physical records. These records are the property of Honolulu DNA Paternity Testing, but the information belongs to you.

We use health information to provide DNA testing services, including identity verification, sample collection, chain-of-custody documentation, laboratory testing, and reporting of results.

B. When We May Not Use or Disclose Your Health Information
