THIS NOTICE DESCRIBES HOW HEALTH INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION.

Honolulu DNA Paternity Testing understands the importance of privacy and is committed to maintaining the confidentiality of your health and identifying information. We create and maintain records related to DNA collection, identity verification, testing services, and results, and we may receive such information from other authorized sources.

We use this information to provide DNA testing services, comply with legal and accreditation requirements, obtain payment for services when applicable, and operate our business in compliance with federal and state law.

We are required by law to:

• Maintain the privacy of your Protected Health      Information (PHI)
• Provide this Notice of our legal duties and privacy      practices
• Notify affected individuals following a breach of      unsecured PHI

This Notice describes how we may use and disclose your information, your rights regarding that information, and our legal obligations.

A. How We May Use or Disclose Your Health Information

We collect health and identifying information about you and store it in secure electronic and physical records. These records are the property of Honolulu DNA Paternity Testing, but the information belongs to you.

The law permits us to use or disclose your health information for the following purposes:

1. Testing Services

We use health information to provide DNA testing services, including identity verification, sample collection, chain-of-custody documentation, laboratory testing, and reporting of results.

We may disclose information to employees, contractors, and AABB-accredited laboratories involved in providing these services.

2. Payment

We may use and disclose health information to obtain payment for services provided to you when applicable. This may include disclosures to insurers, payers, or other entities involved in payment processing.

3. Operations

We may use and disclose health information to operate our business, including:

• Quality assurance and compliance
• Accreditation, audits, and legal services
• Business planning and administrative activities

We may share information with business associates (such as laboratories or document services) who perform services on our behalf. All business associates are required by contract to safeguard your PHI.

4. Appointment Reminders

We may use and disclose health information to contact you regarding appointments or testing instructions. Messages may be left by voicemail, text, or email unless you request otherwise.

5. Communication With Family or Representatives

We may disclose health information to a personal representative, legal guardian, or individual involved in your care or payment for services, unless you instruct us otherwise.

In emergency or disaster situations, disclosures may be made as necessary to coordinate care or notifications.

6. Marketing

We may provide information about services related to DNA testing without receiving payment for such communications.
We will not use or disclose your health information for marketing purposes involving payment without your written authorization.

7. Sale of Health Information

We do not sell your health information without your prior written authorization. Any authorization will disclose whether compensation is involved.

8. Required by Law

We may use or disclose health information when required by federal, state, or local law, including reporting obligations and legal proceedings.

9. Public Health & Oversight

We may disclose health information to public health or oversight authorities as required for audits, inspections, licensure, accreditation, or compliance activities.

10. Judicial and Administrative Proceedings

We may disclose health information in response to court orders, subpoenas, or lawful requests, consistent with applicable legal requirements.

11. Law Enforcement

We may disclose health information to law enforcement officials when required by law, including for identification purposes or compliance with legal process.

12. Coroners

We may disclose health information to coroners or medical examiners as required by law.

13. Public Safety

We may disclose health information to prevent or lessen a serious and imminent threat to health or safety.

14. Specialized Government Functions

We may disclose health information for government functions, including immigration DNA testing requested by U.S. Citizenship and Immigration Services (USCIS).

15. Change of Ownership

If Honolulu DNA Paternity Testing is sold or merged, your records may transfer to the new owner, who will continue to honor this Notice.

16. Breach Notification

In the event of a breach of unsecured PHI, we will notify you as required by law. Notification may be provided by mail, email, or other appropriate means.

B. When We May Not Use or Disclose Your Health Information

Except as described in this Notice or permitted by law, we will not use or disclose your health information without your written authorization.
You may revoke an authorization in writing at any time.

C. Your Health Information Rights

You have the right to:

• Request restrictions on certain uses or      disclosures
• Request confidential communications
• Inspect and obtain copies of your health      information
• Request amendments to your records
• Receive an accounting of disclosures
• Obtain a paper or electronic copy of this      Notice

Requests must be submitted in writing and may be subject to reasonable fees and legal limitations.

D. Changes to This Notice of Privacy Practices

We reserve the right to amend this Notice at any time.
Revisions will apply to all PHI we maintain and will be posted on our website and available upon request.
